Through Your Healthcare Provider
Your healthcare provider can order the test on your behalf by completing and submitting the physician order form through our secure portal. Once we receive the order, we’ll ship the test kit directly to your provided address.

Directly by Patients
If you prefer to order the test yourself, you can do so by:

Fill out the online patient order form with your details and your medical provider’s prescription. Submit the form, and we will process your order.
By Phone:
